To become a Clinton County Humane Society (CCHS) foster parent, I understand that I will be required to abide by the following conditions. I agree to fulfill the duties and responsibilities of an CCHS foster parent outlined here within and to abide by the policies and procedures of the foster care program. Further, I understand:
Foster animals are the property of CCHS and remain so until ownership is transferred at the time of adoption.
I agree to fully read any foster care manual provided and complete any online orientations which will be sent to me after I have been accepted as a foster parent.
I agree to attentively care for the animal(s) as a companion animal being prepped for adoption and to bring the animal(s) back to CCHS for any necessary medical treatment, progress checks, vaccinations, spay/neuter surgery, and adoption.
Foster parents are required to provide transportation to bring foster animal(s) into the shelter for checkups and regular vaccinations.
The length of time each animal needs in foster care varies, and CCHS may request that an animal stay in foster care for a longer amount of time. If necessary, I may return my foster animal at any time, for any reason; I will do my best to contact the foster care team prior to returning the foster animal so accommodations can be arranged.
CCHS has the right to request/demand the return of the animal at any point in time.
I agree not to foster animals from any other organization while I have CCHS foster animals, unless granted specific approval by the foster care team.
Foster animals are not to be left in the care of anyone outside of the foster parent’s household without prior approval from the foster care team.
Adoption inquiries must be directed to CCHS foster care team.
Foster animals are not eligible for adoption until approved by our medical team.
Potential adopters must meet CCHS adoption criteria and pay all applicable adoption fees.
CCHS has the right to deny an adoption based on information provided by the potential adopter.
All animal related ordinances apply to CCHS foster animals, including but not limited to leash and pooper scooper ordinances.
I must always keep any provided rabies and/or foster care ID tags on any canine over 4 months of age and the tags must be returned with the foster animal.
Any time a CCHS canine is taken outside, the canine must be secured with 2 separate leashes attached to 2 separate points of contact (2 collars or a collar and a harness set are considered appropriate points of contact).
I will not allow foster dogs or puppies to have any unsupervised off-leash time outdoors.
Foster animals are not to be taken to public places such as pet stores or off leash dog parks, unless explicit permission is given by the foster care team.
Puppies under 6 months old are never to be taken to public places or allowed to walk on the ground (this is done to prevent the spread of the deadly parvovirus).
When transporting CCHS animals, do not roll the vehicle’s windows down more than 2 inches.
CCHS animals are not to be left unattended in a vehicle under any circumstance.
Foster animals are not to be transported out of state.
Foster animals are not to be relocated to a new home address without prior approval from the foster care team.
It is recommended to keep foster animals separate from your own pet(s) to help contain the spread of any potential illness foster animal may be harboring.
I acknowledge that I have been advised to quarantine my foster animal away from my owned animals.

I agree to release, discharge, indemnify and hold harmless the Clinton County Humane Society (CCHS), a nonprofit 501(c)(3) organization in Ohio, its officers, directors, volunteers, and employees for any and all claims, demands, losses, costs, liabilities, settlement agreements, damages, expenses and suits at law or in equity that arise out of my performing services for CCHS, its officers, directors, volunteers, or employees. I recognize that in handling animals while performing services for CCHS, there exists a risk of injury including, but not limited to, personal physical harm. On behalf of myself, my heirs, assignees, guardians, and personal and legal representatives and executors, I hereby release, discharge, indemnify, and hold harmless CCHS, its officers, directors, and employees for any and all claims, demands, losses, costs, liabilities, settlement agreements, damages, expenses connected with my services to the humane societyociety whether caused directly or indirectly by any negligence (active or passive) attributable to CCHS, its officers, directors, volunteers, or employees. I acknowledge that I am advised and encouraged to consult with my physician regarding: tetanus vaccinations, rabies prophylaxis, and possible allergic reactions. I understand the above and agree to assume all responsibility for this animal(s) and its actions and will assume all liability.
I have read and agree to the Waiver and release
